Crosswalk 2014 to the Redesigned PCMH Program
NCQA released a crosswalk of the 2014 Standards to the redesigned PCMH program (version 6, updated July 2020). The 2014 Elements and Factors are listed along the left-hand column, next to the corresponding PCMH (v6) criteria, which are similar in intent and requirements.
