Designing a PCMH development plan. There are seven suggested steps:

1. Convene PCMH development TEAM
2. Schedule regular PCMH development TEAM meetings
3. Determine goals for PCMH development
4. Complete a Readiness Assessment and identify gaps in PCMH core capabilities
5. Action Planning – Identify a Pathway to PCMH Implementation
6. Develop a workplan and timeline to accomplish your goals, including responsibilities for TEAM members
7. Engage your entire clinic team, Re-assess status regularly, Celebrate successes!
